Applicant must have a minimum 3.0 GPA, be enrolled full-time in an accredited U.S. dental hygiene school with an active chapter of the Sigma Phi Alpha Dental Hygiene Honor Society, and complete the FAFSA or the Family Financial Service form of the ACT.
